As we discussed, I know that this may seem silly or unnecessary to you, but I really want you to give serious consideration to this matter, as it is very important to me.
I value our relationship very much and I certainly view it as voluntary, consensual and welcome, and I have always felt that you feel the same. I know that sometimes, however, an individual may feel compelled to engage in or continue a relationship against their will and out of concern that it may affect their job and/or working relationships.
It is very important to me that our relationship be on an equal footing and that you be fully comfortable that our relationship is at all times fully voluntary and welcome. I want to assure you that under no circumstances will I allow our relationship or, should it happen, the end of our relationship, to impact on your job or our working relationships. Though I know you have received a copy of our company’s sexual harassment policy, I am enclosing a copy so that you can read and review it again. Once you have done so, I would greatly appreciate your signing this letter below, if you are in agreement with me.

I have read this letter and the accompanying sexual harassment policy and I understand and agree with what is stated in both this letter and the sexual harassment policy. My relationship with [John Doe] has been and is voluntary, consensual and welcome. I also understand that I am free to end this relationship at any time, and in doing so, it will not adversely impact on my job.

INFORMED CONSENT ACKNOWLEDGMENT
Both parties acknowledge the following:
1. [John Doe] is presently employed by [Acme Incorporated] as a [Supervisor].
2. [Jane Dee] is presently employed by [Acme Incorporated] as a [Specialist].
3. [Jane Dee] is not presently under the direct supervision of [John Doe], nor has she ever been. Although the professional obligations and work responsibilities of [John] and [Jane]occasionally involve interaction on a professional level, the daily functions of their jobs, and related duties and tasks do not require, necessitate or provide occasion for such interaction.
4. [John Doe] and [Jane Dee] have mutually consented, both individually and collectively, to pursue and become involved in a social and/or amorous relationship (“social relationship”) with each other.
5. [John Doe‘s] desire to actively pursue and become involved in this social relationship is completely and entirely welcome, voluntary, consensual and is unrelated to [Acme Incorporated], his professional or work-related responsibilities or duties, or his and [Jane Dee’s]respective positions in the company or business relationship to each other.
As of the date of this acknowledgment and agreement, [John Doe] acknowledges that his consensual social relationship with [Jane] will not result in or cause a violation of [Acme Incorporated’s] policy on sexual harassment or any applicable state or federal laws on sexual harassment. [John] acknowledges that his agreement not to violate [Acme Incorporated’s]policy on sexual harassment applies to all his interactions with [Jane], whether it stems from, or arises out of his social relationship with [Jane] or business-related interactions.
